Attention parents of all students: Please remember the rules for medications at school. Medications should be picked up before or on the last day of school. If it is a prescription medication, an adult needs to pick it up. Consent forms are only good for one school year. At the beginning of each school year, all new consent forms will be needed, along with all new medications, OTC and prescriptions. That means for an over-the-counter medication we will need parental or guardian consent. Prescription medications will need a doctor’s order as well as parental/guardian consent. If an over-the-counter medication is sent to school it has to be in the original container. We don’t have to have a whole bottle but we do have to receive it in the original container. A student can only take medication at school with written or verbal consent.
It is very important that the school has proper telephone numbers to reach a student’s parents/guardian. If you change your phone number please call and give the new number to the school. When the new school year begins each student will bring home a Student Information Sheet. Please fill it out and return it to the school nurse. Please list any medical problems/concerns that your child may have. Please list any medications they take on a regular basis. Please make sure any allergies are listed. If you mark a medication is needed for a medical condition at school please make sure we have that medication and proper doctor’s order and parental/guardian consent. This form is two sided, please read and fill out each side, sign, and date the back of the form.
Students entering the 7th grade each year are required by DHEC to have a TDAP Vaccine. This vaccine can be administered at a doctor’s office or Health Department. The Health Department administers vaccines on Tuesdays and appointments are required. Once the vaccine has been obtained, we need a copy for the student’s permanent record.
