LONG MIDDLE SCHOOL NURSE

Students entering the seventh grade are now required to have an additional Tdap (tetanus, diphtheria, and pertussis) vaccine in order to attend school.  It is now mandatory that the student receives this vaccine after his/her seventh birthday.  We must have a copy of your child's up-to-date shot record provided at the school and it will need to include the Tdap vaccine.

Please check your child's shot record to see if he/she has received this Tdap vaccine.  If your child has not received this vaccine you need to make arrangements for your child to receive this shot.  If you have any questions please call me.
